That version is modded by Sound Pro.  Denecke only makes it with XLR outs.

How about the Beachtek DXA-6A Audio Adapter

This one should not have that problem right? Also there is another version of it, the "DXA-6HD Dual Hi-Definition Audio Adapter" which uses "Dual Hi-Definiton XLR Inputs". Do you suppose that makes a difference to the sound quality?

Apart from the SONY XLR-1, that is the only option I could find for XLR phantom inputs to 1/8th output which may be wired correctly. Are there others?

I would assume that most mics will work with the 1/8" modded PS-2 actually.  I've had no probs with Neumann KM184 and Schoeps MK4/CMC6 personally, and I rather suspect that many mics will work fine as well.  If you buy it from Sound Pro, you can always return it to them within 30 days I believe if it doesn't work with whichever mics you own.
