Depends on the band but I will run the 9100 at max occasionally. I usually keep it around 2-3 O'clock most of the time though and use the dial on the M10 to adjust the rest.
I will say that I just taped some of the ACM awards shows in Las Vegas a week or so ago and I ran the 9100 at max and the M10 @ 3 on the dial and came away with some stellar recordings. I think it just depends on the situation and how daring you want to be with the hopes of not overloading or getting distortion.
I tend to run things a little hot most of the time and have never had a issue...knocks on wood...

I think I got the recommendation to run the CA9100 at max from Chris Church (but I'm not 100% sure about that). I record at 96/24, so I generally have the dial the signal on the 09 down pretty far and increase volume in post if necessary.

Chris recommends running the 9100 at 80-90%, and lower for really loud shows.
