The name implies the first but I'd still like to get a definite answer on how those boxes manage to provide different voltage outputs... I don't have the possibility to disassemble and look into one on my own. Do they use voltage regulators, voltage dividers or even a mixture of both?

generally a cheap IC VR or LDO VR.  I am sure there is a mixture of fixed and variable VRs depending on the maker
