V3 > SD722 Anyone ever see this happen?

I was running a digital signal from a lunatec V3 into a Sound Devices 722 via XLR cable last week.  At one point the V3 clipped (left clip light on, musicians bad guess on what was their loudest part).  On the 722 recording in Wavelab I see clipping at -6 dB, not 0 dB.  I am running the latest firmware for the 722 (2.61).  I have tried feeding the V3 signal to a Fostex FR-2 and also again to the 722, and can't reproduce the problem.  Does anyone have any idea what I could have been doing wrong here to lose a bit?

At one point the V3 clipped

V3's clip?  Is that up there with the Cubs winning the Series?
