i ran the coax. here is a copy from grace design and their thoughts on the problem

Thanks for the email. Bummer that you are
experiencing this issue. Do
try to isolate the problem such that we can
either rule out the preamp
or localize the failure to it.
From what you have described, I gather that the
noise you are getting
resembles digital drop-outs. Is this correct?
Is it happening on both
channels simultaneously? Is it occurring
regularly?
To isolate the problem, I would start by ruling
out the record deck as
this is the only other device to be touching the
signal in the digital
realm. First, verify the integrity of your
S/PDIF cable. A flaky cable
could certainly cause this issue. Next if you
have another deck,
connect it via S/PDIF and see if you get the same
results.
If you do, then it is pretty safe to assume that
your hhb deck is not
the source of the problem. At this point, your
mics/cables are the only
other variable. From the way you described the
problem however, it
doesn't sound like a microphone issue. To be
sure however, if you have
another mic(s) and cable(s) hook those up to see
if the problem still
exists.
Please let me know your findings. We have seen
one other V3 that the
user was getting a random (single) click like
once every 3 hours of
recording. This issue was narrowed down to an
intermittent cold solder
joint on one of the digital components.
