The old DIY thread: http://taperssection.com/index.php?topic=106354.msg1419931#msg1419931 (http://taperssection.com/index.php?topic=106354.msg1419931#msg1419931)
Anyone know where else or what part#'s you need to build a new one? Thanks.

You can easily make one by cutting the ua5's wall wart power cord and the dvd battery's output cable and splicing the dvd battery connector to the ua5's power connector. Polarity is always an issue -- If I recall, the UA5's polarity is "backward" from the norm and requires splicing the red wire (positive) from one cable to the black/white (negative) wire from the other. Crucial to use a voltmeter to verify polarity before using.

If I may make a suggestion. Those adapt a plugs are crap. I had a few break on me, just under the strain relief. If your handy with soldering, you'll get longer lasting results if you just buy the bare plug:
i.e. (and you can get these anywhere, jameco)
M plug:

No item for B plug, but it might be this one:
http://www.radioshack.com/product/index.jsp?productId=2102598 (http://www.radioshack.com/product/index.jsp?productId=2102598)
I have bought the B one many times, just as a bare plug. It was not labeled as B, just as the size.
just my .02
