I need an ad/da converter with XLR Phono Toslink USB in and outputs...
Any suggestions ?

Sound Devices  usbpre2. It has all that and more assuming that by phono you mean rca ins and out not a phono preamp.

You need an A/D and D/A converter with every input and output known to man?  For what purpose?  What is your application?  First, you are looking for something that in all likelihood does not exist.  Second, if this object does exist, the odds that it can be powered in the field are very low.  Third, even if these two conditions are satisfied, such a piece of equipment may cost a significant amount of money.

What are you trying to do?  What do you already have?  What is your application?

I will used it as a DA converter for my CD player. XLR and RCA
To digitalise analoge cassettes onto a computer. RCA to USB
DAT Toslink to make a copy of tapes onto a computer.
 
Sound Devices  usbpre2 looks nice.
I can also use it as a mic amp for my B&K 4035.

Have anyone tried the Sound Devices  usbpre2 ?

Yes, there are quite a few members here familiar with the unit. Here's links to the "SD USBPre2" threads:

For the needs you describe it seems like the SD would be a good fit. It's portable and has most (if not all) the inputs you mentioned.
